If you have been sick for seven calendar days in a row you can self–certify your illness using a SC2 form. You can obtain this form from your employer or by visiting the HMRC website. If you are unwell for more than seven days you are advised to contact the Practice to arrange a medical certificate.
Statement of Fitness for Work
The Doctor will provide you with a Statement of Fitness for Work (‘Fit Note’) if you are still not well enough to work. Your employer will most likely request this statement as evidence to support payment of Statutory Sick Pay (SSP).
Sick line clinics run on Tuesdays, Wednesdays and Fridays. When you speak to a member of reception, your name will be booked in to the next available clinic – you do not need to come to the Practice.
You will be asked for details of your condition and how long you require the Fit Note for. The Doctor may issue a Fit Note for you, or may wish to discuss the request with you first and so the Receptionist will also take a contact number. Any completed Fit Notes can be collected at the Practice after 3.30 pm. If the Doctor does not contact you, assume that the Fit Note has been completed.
